Our mission
Threshold Ledger exists to give readers a clear, honest picture of one of the most consequential decisions in opioid care: whether, and how, to move between methadone and ibogaine. The space is loud with both hype and fear; our job is to synthesize evidence, protocols and ethics into plain language that helps people ask clinicians better questions.
How we report
We work from primary and authoritative sources — peer-reviewed literature, published clinical protocols, and official policy — and we foreground harm reduction throughout. Cardiac safety, informed consent and medical supervision are treated as non-negotiable. Where the evidence is thin or contested, we say so plainly.
What we are not
We are not a clinic, a treatment provider, or a medical authority, and nothing here is medical, legal or financial advice — nor is any page a protocol to follow. Decisions about opioid treatment belong with qualified clinicians who know your history.
